Prognostic factors in patients with disseminated nasopharyngeal carcinoma

Résumé

Purpose. To identify the prognostic factors for patients with disseminated nasopharyngeal carcinoma (DNC). Material and methods. The study concerned 80 patients with DNC treated between 1995-2007. Forty of them received cisplatin based salvage chemotherapy, other patients were treated with palliative intent. Results. One-year overall survival rate in the whole group was 31%, and it was 55% for patients treated with a salvage chemotherapy and 7% for receiving palliative care (p=0,0001). Both uni- and multivariate analysis demonstrated poor performance WHO/ECOG status and liver metastasis to be statistically significant for unfavorable outcomes. One-year overall survival rates for patients with WHO/ECOG values 0-1, and 2-3 were 50% and 10% respectively (p=0,0005). Liver metastasis decreased one-year overall survival from 34% to 10% (p=0,0009). There was no significant impact on overall survival of the other studied factors as age, gender, dissemination free interval, number of metastases and prior chemotherapy. Conclusions. Patients with DNC, free of liver metastasis and having a good performance status present the most favorable outcome. This group of patients may benefit from an aggressive salvage chemotherapy. © ONKOLOGIA I RADIOTERAPIA.
