L'agglomération de Constantine (Algérie) au prisme de la territorialisation sanitaire
Résumé
The Algerian healthcare system is characterised by a duality in its organisation: public, directly dependent on the central power and with a strong hierarchy; private with freedom of installation both in terms of specialties and in terms of geography. Through the example of the wilaya of Constantine, which shelters the 3rd city of the country –only metropolis of the interior–, the health territorialisation is examined at fine levels, which underlines that what is at stake for this health system is not just a North/South and urban/rural differentiations. In this highly urbanised wilaya, the recent dynamics of the private sector bring to the fore the doubts of the inhabitants vis-à-vis the public health sector, but also the inequalities of access to various care in their social and territorial dimensions, even if the practices declared by these same inhabitants seem more nuanced.
Le système de soins algérien se caractérise par une organisation duale : une partie publique dépendant directement du pouvoir central et fortement hiérarchisée ; une partie privée jouissant d’une liberté d’installation tant en termes de spécialités qu’en termes géographiques. À travers l’exemple de la wilaya de Constantine, qui abrite la 3e ville du pays – seule métropole de l’intérieur –, la territorialisation sanitaire est examinée à des niveaux fins, qui soulignent que les enjeux auxquels doit faire face ce système de santé ne se résument pas uniquement à une différenciation Nord / Sud et/ou urbain / rural. Dans cette wilaya très urbanisée, les dynamiques récentes du secteur privé remettent au premier plan les défiances des habitants vis-à-vis du secteur public de santé, mais aussi les inégalités d’accès à des soins variés dans leurs dimensions sociales et territoriales, même si les pratiques déclarées par ces mêmes habitants semblent plus nuancées.
