The therapeutic education in between medical truth and the history of the patient
L'éducation thérapeutique entre vérité médicale et histoire du patient

Le diagnostic médical est posé en termes scientifiques. fondé sur une logique démonstrative selon laquelle le patient adulte atteint d'une maladie chronique, l'asthme dans cette recherche, se trouve dans l'obligation de respecter un certain nombre de normes afin de prévenir le surgissement de crises aiguës.
Du coté médical, la maladie chronique est souvent sans guérison possible, les traitements proposés visent à atténuer les symptômes et à rendre la vie possible. Malgré des séances éducatives destinées à ces patients. nous constatons un non respect de ces consignes. Que pourrait signifier cette « non compliance ». à l'échelle du patient ? En plaçant la réflexion du côté de l'herméneutique, nous allons tenter de comprendre ce que la parole du sujet pourrait révéler. Cette approche dévoilerait-elle au-delà d'une « clinique du signe » quand il s'agit de découvrir la maladie par la preuve, une « clinique du signifiant » (Dubas, 2004) quand il s'agit de découvrir la maladie éprouvée ?
Ce comportement relèverait-il d'un déni ?
Le patient retirerait-il de sa maladie des bénéfices secondaires comme l'a montré Laplantine (1986) ? En quoi l'itinéraire du patient, les épreuves traversées suite à la survenue de la maladie. son histoire éducative. familiale et sociale permettraient-ils de comprendre cette compliance ou cette non compliance?
A la faveur d'une étude portant sur les récits de cinq patients, nous avons tenté d'identifier les modes de fonctionnement, les enjeux identitaires. les histoires propre à chacun. L'orientation méthodologique de cette thèse se situe donc dans le champ d'une démarche clinique,
The medical diagnosis is expressed in scientific terms. based on demonstration logic on which is stated that adult patients with a chronic disease, such as asthma in this research. have to respect certain standards to prevent sudden appearance of acute crisis.
From the medical point of view. a chronic disease is often considered as not curable. the proposed treatments aim to alleviate the symptoms and make life more comfortable. Despite educational sessions provided to these patients. we observe a phenomenon of non-compliance with the instructions. What could this "non-compliance" mean at the patient's level?
As we give further thought to the interpretative level. we will try to understand \.^,fhat the words of the subject might reveal. VVould this approach also reveal. beyond a 'clinical sign" when it comes to refer to the disease as an evidence. a "clinic of meaning" (Dubas. 2004) when it comes to discover the proven disease? Would this behavior result of a denial? VVould the patients obtain side benefits from their illness as shown in Laplantine's work (1986)? Given the patients itinerary. their experiences endured through the emergence of the disease. their educational. social and family history. shouldn't ,Afe be able to understand this compliance or non-compliance? Taking advantage of a study of the accounts from five patients. ."e have attempted to identify the modes of behavior. the identity issues and the proper histories of each one. The methodological orientation of this thesis is therefore within the scope of a clinical and qualitative approach.
