Suture à ciel ouvert du tendon d’Achille : récupération fonctionnelle au long cours

Résumé

Introduction The incidence of traumatic ruptures of the Achilles tendon has been increasing since the early 2000s. Many surgical techniques (percutaneous, minimally invasive) and non-surgical techniques exist. The objective of our study is to evaluate the functional recovery at one year of recoil after open suture of the Achilles tendon with the Achilles’ Traumatic Rupture Score. Material and methods This was a retrospective monocentric study of the 53 consecutive patients who had surgically treated open-cast traumatic Achilles tendon rupture between 2010 and 2016 at Clermont-Ferrand University Hospital. The primary endpoint was at least one year of functional recovery evaluated by the Achilles Tendon Rupture Score (ATRS). Secondary endpoints included time to recovery, sports recovery, surgical time, scarring, infections and pre-existence of subjective Achillian tendinopathy. The mean follow-up of the series was 47 months (13 to 77 months) with an average operating time of 3 days (0 to 19 days). Results We had no loss of sight. Clinical recovery results were good with an ATRS score at 90% on average (70% to 100%). Surgical complications were only 13.2% of scarring disorders without recurrence of rupture. Discussion The technique of suturing the Achilles tendon is a technique that gives good functional results with a recurrence rate that remains low. However, the skin complications are not negligible and are the major complications of this technique. Early postoperative mobilization could improve the functional outcome.
