La ride orbito-malaire externe: Un nouveau marqueur clinique associé à une majoration du risque cardiovasculaire ?

Abstract : Context: The diagonal earlobe crease (Frank's wrinkle) is well described as a cutaneous marker associated to a major cardiovascular risk. During primary care clinical observations, we have noticed a new cutaneous sign potentially associated. Objective: The " wrinkle orbito-malaire external " (ROME) is described here, for the first time, as a potential clinical marker associated with to a major cardiovascular risk. In this preliminary study, the question was to test such an hypothesis stemming from clinical observations. Method: We have collected clinical and epidemiological data within a 400 patients cohort, from 30 to 80 years old, reported during general practice consulting inclusion. The risk factors and cardiovascular events have been compared with Frank's wrinkle and ROME ranks. A semi-quantitative evaluation has consisted of uni-parametric correlation test between qualitative ordinal variables. Results: In our series, ROME, has appeared as a clinical sign significantly correlated to a major cardiovascular risk and increasing cardiovascular events. This association has been comparable to Frank's wrinkle observations, previously well known as a cutaneous marker of cardiovascular diseases. Conclusion: This preliminary transversal study could deserve multicentrics follow-up studies, with a view to discuting our observations and understanding the possible co-constitution of such wrinkles with systemic atherosclerosis. We emphasize the interest of visible markers for the primary prevention and early screening of cardiovascular diseases.
